In house physical, occupational & speech therapy in two state-of-the-art rehabilitation gyms.
Education
Activities of daily living skills taught in a real world environment.
Home Assessment
Includes a home visit to assess accommodations, arranging for special equipment and, if needed, educating family or other caregivers who may assist upon the patient's return home.
Pain Management
Our team takes a collaborative approach to treat many types of pain such as head and neck; back; arm and hand; and leg pain with many state of the art non-surgical treatments.
Hospice
Hospice care is a type of care and philosophy of care that focuses on the palliation of a chronically ill, terminally ill or seriously ill patient's pain and symptoms, and attending to their emotional and spiritual needs.
Respite Care
Le Rêve also provides respite care as a short-term relief for primary caregivers. We are able to accommodate your loved one for a single night or for several days or weeks.
Memory Care
Le Reve also provides memory care services with a special support team and care plans personalized for each resident. We are experts at managing conditions from mild cognitive impairment and early stage memory loss to Alzheimer’s and late stage memory care and dementia. Our mission is to provide expert memory care in a setting that feels like home rather than a care facility.
